This role offers true 1:1 patient care with time to focus, not rush. You will have autonomy in the field with leadership that trusts your judgment. Our systems and EMR are built for real-time documentation, not after-hours catch-up. We are a company large enough to be stable, but still locally owned and clinician-focused.

Requirements

• Active Minnesota Registered Nurse (RN) license
• Experience in home health, med-surg, rehab, or clinical nursing
• Strong clinical judgment and independence
• A nurse who values connection, autonomy, and meaningful work
• Reliable transportation and willingness to travel locally

Responsibilities

• Provide skilled nursing care in homes and senior communities
• Deliver treatments such as wound care, medication management, and assessments
• Build relationships with patients and families over time
• Coordinate care with physicians, therapists, and interdisciplinary teams
• Document in real time and stay focused on patient care
