Requirements

• Active Minnesota Registered Nurse (RN) license.
• Experience in home health, med-surg, clinic, rehab, or hospital nursing.
• Strong clinical judgment and independence.
• Comfortable working in a field-based role.
• Reliable transportation.

Responsibilities

• Provide skilled nursing visits in homes and care communities.
• Perform assessments, wound care, and medication management.
• Educate patients and families on care plans and recovery.
• Coordinate with physicians, therapists, and care teams.
• Maintain accurate, timely documentation.
