About The Position
Home Health Care, Inc. is seeking a dedicated RN Visit Nurse to provide 1:1 patient care in homes, assisted living, and community settings. Unlike other agencies that may stretch nurses thin or leave them unsupported, we prioritize supporting our nurses so they can focus on quality patient care. Requirements
- Active Minnesota Registered Nurse (RN) license.
- Experience in home health, med-surg, rehab, or clinical nursing.
- Ability to work independently while staying connected to a team.
- Strong communication and patient education skills.
- Reliable transportation and willingness to travel locally.
Responsibilities
- Provide skilled nursing visits in patient homes and care settings.
- Deliver hands-on care including wound care, medication management, and assessments.
- Build consistent relationships with patients and families.
- Coordinate care with physicians, therapists, and interdisciplinary teams.
- Educate patients and caregivers on care plans and recovery goals.
- Complete accurate, timely documentation.
