About The Position
This is a field-based nursing role where nurses have more control over their day, better communication, and less burnout. The position offers autonomy without isolation, with accessible leadership and consistent support from scheduling, clinical, and operations teams. It is with a locally owned company that prioritizes long-term team retention and clear expectations. Nurses start their day from home and conduct 1:1 visits with patients, allowing time to think and assess. The role involves providing skilled nursing care, building relationships with patients and families, coordinating with healthcare teams, educating patients and caregivers, and completing real-time documentation.
Requirements
- Active Minnesota Registered Nurse (RN) license.
- Experience in home health, med -surg, rehab, or clinical nursing.
- Strong clinical judgment and ability to work independently.
- Organized, self -directed, and comfortable in a field role.
- Reliable transportation and willingness to travel locally.
Responsibilities
- Travel to patient homes and care settings throughout your assigned territory.
- Provide skilled nursing care, including wound care, assessments, and medication management.
- Build relationships with patients and families over time.
- Coordinate with physicians, therapists, and interdisciplinary teams.
- Educate patients and caregivers to support recovery and independence.
- Complete real-time documentation to stay ahead of your day.
