Registered Nurse (RN) | Home Health Visit Nurse
About The Position
Home Health Care, Inc. is seeking an experienced RN Visit Nurse who enjoys the independence of working in the field while still having an established clinical team behind them. This role offers a unique opportunity to practice nursing in a rewarding environment where the patient's home becomes the primary setting for care. The RN Visit Nurse will travel to patients' homes to provide skilled nursing care, conduct comprehensive assessments, and make critical clinical judgments that can prevent hospital readmissions. This position is ideal for nurses who are confident in their clinical skills, enjoy patient education, and are looking for more autonomy in their practice.
Requirements
- Active Minnesota Registered Nurse (RN) license.
- Strong nursing assessment and critical-thinking skills.
- Previous experience in home health, hospital nursing, rehabilitation, skilled nursing, or other clinical settings.
- Confidence working independently in the field.
- Strong patient and family communication skills.
- Excellent organization and time-management abilities.
- Reliable transportation and willingness to travel throughout an assigned service area.
- A nurse who takes ownership of their patients and follows through.
Nice To Haves
- Previous home health experience is valuable.
- Nurses from other clinical environments with the right combination of independence, assessment skills, and patient-care experience are encouraged to connect.
Responsibilities
- Travel throughout an assigned area providing skilled nursing care directly in patients' homes.
- Conduct comprehensive nursing assessments.
- Perform wound care and wound assessment.
- Conduct medication reconciliation and management.
- Administer injections and other skilled nursing procedures.
- Monitor chronic and acute medical conditions.
- Educate patients and caregivers on treatment plans and health management.
- Identify changes in patient condition and address them proactively.
- Communicate concerns and updates with physicians.
- Coordinate with PT, OT, SLP, social work, and other members of the care team.
- Help patients understand how to manage their health safely at home.
- Complete timely documentation in Homecare Homebase (HCHB).
