About The Position

Home Health Care, Inc. is seeking an experienced RN Visit Nurse who enjoys the independence of working in the field while still having an established clinical team behind them. This role involves traveling throughout an assigned area to provide skilled nursing care directly in patients' homes. The position offers a unique opportunity to practice nursing with a focus on individual patient needs, clinical judgment, and patient education, making a significant difference in preventing hospital readmissions.

Requirements

  • Active Minnesota Registered Nurse (RN) license
  • Strong nursing assessment and critical-thinking skills
  • Previous experience in home health, hospital nursing, rehabilitation, skilled nursing, or other clinical settings
  • Confidence working independently in the field
  • Strong patient and family communication skills
  • Excellent organization and time-management abilities
  • Reliable transportation and willingness to travel throughout an assigned service area
  • A nurse who takes ownership of their patients and follows through

Responsibilities

  • Provide skilled nursing care directly in patients' homes.
  • Conduct comprehensive nursing assessments.
  • Perform wound care and wound assessment.
  • Conduct medication reconciliation and management.
  • Administer injections and perform other skilled nursing procedures.
  • Monitor chronic and acute medical conditions.
  • Educate patients and caregivers on health management.
  • Identify changes in patient condition.
  • Communicate concerns and updates with physicians.
  • Coordinate with PT, OT, SLP, social work, and other members of the care team.
  • Help patients understand how to manage their health safely at home.
  • Complete timely documentation in Homecare Homebase (HCHB).
