About The Position

Jordan Health is seeking an RN Triage to work collaboratively in a Family Medicine practice setting, under the guidance of the Director of Nursing. The role is primarily responsible for identifying and proactively managing patients’ care needs by providing assessment, care coordination, advocacy, and coaching. The position supports high-quality, consistent, competent, patient-centered care; health outcome goals; financial and regulatory objectives; and access and engagement goals.

Requirements

At least 1 year of nursing experience.
Current New York State RN license without negative actions.
Current provider BLS/CPR certification through the American Heart Association.
Infection Control Certification.
Strong computer skills.
Excellent customer service skills by phone and in person.
Ability to work well with others in a multidisciplinary team.
Ability to organize and multitask.

Nice To Haves

Experience in urgent care, an emergency department, pediatrics, or OB/GYN.
Bachelor of Science in Nursing (BSN).
