About The Position

The RN Team Leader Case Manager supports safe, effective patient care and discharge planning for the adult population served by Sovah Health – Danville. The role works collaboratively with nursing staff and a multidisciplinary team, including Utilization Review, Case Management, and Social Work, to evaluate patients’ medical care and coordinate personalized support throughout their care journey. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with a weekend rotation. The hourly pay range is $34.04–$45.96.

Requirements

• Active Virginia single-state RN license or compact license; if relocating, licensure must be updated to the new Primary State of Residence before employment begins.
• Associate’s degree in nursing.
• Previous Case Management experience.

Responsibilities

• Consult regularly with nursing staff and the multidisciplinary team to evaluate patient status and the appropriateness of medical care, including admission, length of stay, transfer, and discharge.
• Monitor patient and family satisfaction and respond to questions and complaints from patients, family members, and payors regarding care.
• Participate in discharge planning, including coordinating transfers to other facilities and coordinating community resources.
• Provide discharge education and resource referrals to patients.
• Review charts to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
