About The Position

The RN Case Manager (RNCM) coordinates patient and family interactions with Tidewell Hospice throughout the hospice experience, beginning at admission. The RNCM creates and maintains the patient’s plan of care and provides nursing services with attentive listening, observation, communication, and sensitivity in emotionally intense situations. The nurse reviews and updates care plans, physician orders, pharmacy profiles, and other records. The role also includes educating patients, family members, and other caregivers about the disease process and decline, available palliative interventions, the dying process, and safety practices. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with coverage in North Port and Englewood. The position is with Tidewell Hospice, part of Empath Health, a not-for-profit healthcare organization serving communities across Florida.

Requirements

Graduate of an approved nursing program.
Current Florida RN license.
At least one year of experience providing direct patient care in an acute-care hospital environment.
Participation in the seasonal influenza program is a condition of employment.
