About The Position

Elara Caring is hiring a full-time Home Health RN Case Manager to serve patients throughout the Northampton-Easthampton territory. The role provides skilled nursing care in patients’ homes, helping them recover, manage their health, and remain independent. The nurse guides patients through home health care from assessment to discharge while managing an assigned caseload and coordinating care with physicians and other clinicians.

Requirements

• Current, unrestricted Massachusetts RN license.
• At least one year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to work independently and travel throughout the assigned territory.
• Valid driver’s license, current auto insurance, and reliable transportation.
• Ability to sit, stand, bend, lift, and move intermittently, and lift 50–100 pounds.

Responsibilities

• Provide skilled nursing visits in patients’ homes.
• Assess patients and guide their home health care through discharge.
• Develop and update patient care plans.
• Manage an assigned caseload.
• Coordinate care with physicians and other clinicians.
• Teach patients and caregivers to manage medications, treatments, and health conditions at home.
