About The Position

ComForCare Home Health Care - Decatur is seeking a compassionate and driven RN Home Health Case Manager to provide high-quality, person-centered care to clients in their homes in Decatur, Georgia. The role focuses on promoting clients’ independence through community-based care. The Case Manager assesses clients, develops and manages individualized care plans, coordinates care across the care continuum, and collaborates with physicians, therapists, and other interdisciplinary team members. The position also includes documenting care, educating clients and families, and monitoring and adjusting care plans as appropriate.

Requirements

Active and unrestricted RN license in the state of Georgia.
Minimum of 1–2 years of clinical nursing experience.
Experience with case management, care coordination, and interdisciplinary collaboration.
Excellent clinical assessment, critical thinking, and problem-solving skills.
Valid driver's license and reliable transportation for home visits in the Decatur, Georgia area.
Strong communication skills and a compassionate, patient-first approach to care.

Responsibilities

Conduct comprehensive client assessments and develop individualized care plans for home health clients.
Coordinate and manage all aspects of client care as the primary Case Manager across the care continuum.
Collaborate with physicians, therapists, and interdisciplinary team members to support optimal client outcomes.
Document client progress, care plans, and visit notes in compliance with regulatory and agency standards.
Educate clients and family members about diagnoses, medications, and home safety practices.
Monitor and evaluate client progress, adjusting care plans as clinically appropriate.
