Home Health RN Case Manager
About The Position
Interim HealthCare of Scranton is hiring a Registered Nurse RN Case Manager to provide home health care in Scranton, Pennsylvania, and surrounding areas. The nurse will manage an assigned caseload, provide one-on-one patient care in the community, assess and reassess patients, develop and update care plans, coordinate care with physicians and the healthcare team, complete compliant documentation, and educate patients and families. The agency describes itself as locally owned and serving Scranton, Lackawanna County, and surrounding NEPA communities, with consistent patient assignments, manageable caseloads, and clinical support. The position pays $38–$40 per hour, with weekly pay and direct deposit. The regular schedule is Monday through Friday, 8:30 AM to 5:00 PM, with participation in an on-call, weekend, and holiday rotation.
Requirements
- Active Pennsylvania Registered Nurse (RN) license.
- At least 1 year of home health RN experience.
- CPR certification.
- Valid driver’s license and reliable transportation.
Nice To Haves
- Case Manager or Case Management experience preferred.
