About The Position

Moments Hospice is seeking an RN Team Director to lead a compassionate hospice care team and coordinate holistic care for patients and families navigating end-of-life journeys. The role works collaboratively with an interdisciplinary team—including nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers—to ensure care respects each patient’s needs and beliefs. The director serves as a central communication hub for families, physicians, and other providers, and helps ensure seamless care coordination. The position offers autonomy and variety, with training programs, on-call support, clear expectations, and supportive leadership. The role is on the road.

Requirements

• Registered Nurse (RN) license.
• Minimum of 3 years of healthcare experience.
• Valid driver's license.

Nice To Haves

• Hospice or related healthcare experience is preferred.
• Bachelor's degree is strongly preferred.
• Leadership or management experience is a major plus.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and other care professionals.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings and document patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling and performance reviews, and foster a positive work environment.
• Coordinate communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.
