RN Team Director (Registered Nurse)
About The Position
Moments Hospice is hiring an RN Team Director (also described as a Hospice RN Clinical Manager/Team Director) to lead hospice care delivery and an interdisciplinary team. The role coordinates holistic care for patients and families facing end-of-life journeys, while ensuring care respects individual needs and beliefs. The director serves as a central point of communication among families, physicians, other providers, and the care team, and helps maintain seamless care coordination. The position includes managing a patient caseload as needed and participating in after-hours and on-call duties. The job is on the road. The compensation range is $90,000–$100,000 per year.
Requirements
- Registered Nurse (RN) license.
- At least 3 years of healthcare experience; hospice or related experience is preferred.
- Valid driver's license.
Nice To Haves
- Bachelor's degree strongly preferred.
- Leadership or management experience is a major plus.
- Experience in hospice or related settings.
Responsibilities
- Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and other care professionals.
- Ensure adherence to hospice regulations and standards.
- Facilitate communication between medical staff and the care team.
- Lead interdisciplinary meetings and document patient needs and care plans.
- Develop and update patient care policies and emergency procedures.
- Manage staff scheduling and performance reviews, and foster a positive work environment.
- Coordinate communication with families, physicians, and other providers.
- Analyze quality metrics and manage a caseload of patients as needed.
- Participate in after-hours and on-call duties to ensure continuity of care.
- Maximize resource utilization and promote a collaborative work culture.
