About The Position

Moments Hospice is hiring an RN Team Director to lead its interdisciplinary hospice care team and coordinate holistic care for patients and families facing end-of-life journeys. The director works with nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers, while coordinating with families, physicians, and other providers. The role offers autonomy, training programs, supportive leadership, and on-call support. It includes managing a caseload of patients as needed and participating in after-hours and on-call duties.

Requirements

• Active Registered Nurse (RN) license.
• At least 3 years of healthcare experience; hospice or related experience is preferred.
• Valid driver's license.

• Bachelor's degree is strongly preferred.
• Leadership or management experience is a major plus.
• Experience in hospice or related settings.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and other care team members.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings and document patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling and performance reviews, and foster a positive work environment.
• Coordinate communication with families, physicians, and other providers.
• Analyze quality metrics and manage a patient caseload as needed.
• Participate in after-hours and on-call duties to support seamless care.
• Maximize resource utilization and promote a collaborative work culture.
