About The Position

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. The role works with the interdisciplinary team to develop and implement patient- and family-centered plans of care, discuss goals of care, and guide healthcare decisions to support quality of life. The ideal candidate brings energy and motivation, strong managerial, organizational, and interpersonal skills, and the ability to provide precise, effective guidance and support in demanding situations.

Requirements

• Active Kansas or Compact RN License.
• Valid driver’s license and auto liability insurance.

Nice To Haves

• Hospice experience preferred.

Responsibilities

• Provide nursing care, education, and support to patients and families.
• Complete initial and ongoing assessments.
• Coordinate care with physicians and the hospice interdisciplinary team.
