About The Position

Suncrest Hospice is seeking a Registered Nurse to provide care and services to a group of patients on an as-needed (PRN) basis. The nurse visits patients where they reside, including private homes, skilled nursing facilities, and assisted living facilities. As a contributing member of the interdisciplinary team, the RN provides expertise in psychosocial assessments and interventions and knowledge of community resources.

The role involves working closely with patients one-on-one while collaborating with and receiving support from the patient care team. The nurse manages visits, documentation, meetings, and urgent patient needs across the service area.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state of Ohio.
• Ability to work collaboratively in a team environment.
• Ability to work independently on a day-to-day basis.
• Time-management skills to schedule visits, attend required meetings, complete documentation, and respond to urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive, with reliable transportation, a valid driver’s license, and auto insurance.
• Understanding of hospice rules and regulations.

Responsibilities

• Provide care and services for a group of hospice patients.
• Visit patients in private homes, skilled nursing facilities, and assisted living facilities.
• Contribute to the interdisciplinary team by providing expertise regarding psychosocial assessments and interventions and knowledge of community resources.
• Collaborate with nurses, social workers, chaplains, volunteers, and aide leadership.
• Manage visits, documentation, and meetings; respond to urgent needs by prioritizing and triaging multiple concerns.
• Communicate by phone, email, and in writing, and meet with healthcare professionals.
• Travel across the service area to visit patients.
