About The Position

Allay Hospice is seeking a dedicated Hospice Registered Nurse Case Manager to provide expert, patient-centered care to hospice patients and their families in the home. The position covers Sheboygan, Ozaukee, and southeast Calumet counties and is based at the Sheboygan office. The schedule is full time, Monday through Friday, 8:00 a.m. to 5:00 p.m., with participation in an on-call rotation as required by the local branch. The role offers independent work alongside team support, flexible scheduling options, and the opportunity to make a meaningful difference for patients and families.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least one year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Licensed driver with an insured automobile that meets state or organization requirements and is in good working order.
• Current CPR certification in states where required.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create hospice care plans that align with patients’ wishes and goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the on-call rotation as required by the local branch.
