Registered Nurse (RN) Case Manager- Hospice
About The Position
Pathways Hospice is seeking a Registered Nurse (RN) Case Manager to provide and coordinate hospice care for patients and their families in the Northwest, Southwest, and Southeast Houston territories. The role includes overseeing the nursing care team, planning and directing home care services, assessing patient needs, and developing care plans. The nurse collaborates with physicians, therapists, and other healthcare professionals to provide cohesive and comprehensive care, and educates patients and families about health conditions, medications, and lifestyle changes. Clinical notes must be timely, complete, and accurate. This is a PRN role. Night and weekend work is required as patient needs dictate, and the position participates in a weekly on-call rotation. Employees who work a weekend receive the following Monday off; admissions are included. Requirements
- Valid RN license in the State of Texas.
- At least one year of hospice experience.
- Knowledge of home health regulations and general healthcare compliance.
- Excellent communication and people skills.
- Ability to work independently and as part of a team.

- Experience with HomeCare HomeBase (HCHB) EMR.
Responsibilities

- Handle admissions as part of weekend coverage.
