About The Position

Adoration Health is seeking a Hospice Registered Nurse Case Manager to provide direct care and coordinate with the interdisciplinary team to support patients and families. The role is based in Baltimore, Maryland, and covers Baltimore City. The schedule is full time, Monday through Friday, 8 a.m. to 5 p.m.

The nurse completes patient and family assessments, develops and reevaluates plans of care, administers prescribed medications and treatments, and provides counseling and support. The role also involves clinical documentation, communication with physicians, participation in interdisciplinary meetings, and compliance with applicable state and federal requirements.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least 1 year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families.
• Initiate patient plans of care and reevaluate them as necessary.
• Use health assessment data to determine nursing diagnoses.
• Administer medication and treatment as prescribed by the physician in the plan of care.
• Counsel patients and families in meeting nursing and related needs.
• Complete and maintain accurate clinical notes.
• Communicate with physicians and participate in interdisciplinary group meetings to meet patients’ and families’ or caregivers’ medical and emotional needs.
• Ensure compliance with all state and federal legal and regulatory requirements.
• Perform additional responsibilities as assigned.
