About The Position
PruittHealth Hospice is seeking a Registered Nurse Case Manager to provide hospice care to patients in their homes or care facilities. The position serves one of the listed areas in Harnett, Sampson, Cumberland, Hoke, Lee, or Robeson County. Requirements
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
Responsibilities
- Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
- Complete all documentation and records in a timely and accurate manner.
- Follow established policies and procedures related to the scope of practice.
- Maintain continuity of patient care in a hospice setting.
