RN / Registered Nurse Case Manager - Hospice
About The Position
Generations Hospice is seeking a passionate, dedicated Hospice RN to join our team! Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. This role offers a supportive, fulfilling environment where your skills and empathy truly shine. The position features lower patient-to-nurse ratios for meaningful, unrushed hospice care, strong interdisciplinary team support, and leadership that understands hospice nursing. A dedicated internal after-hours triage team supports nurses outside of regular business hours. The role utilizes Homecare Homebase (HCHB) EMR for streamlined documentation. The office is located at 1028 S. Main Street Suite C Fond du Lac, WI 54935, and coverage area requires travel throughout the Fond du Lac (North or Southwest) territory. The schedule is Monday-Friday 8 AM-5 PM.
Requirements
- Graduate of an accredited nursing school with current licensure to practice in state of operation
- One year nursing experience required
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
Nice To Haves
- 3+ years preferred
- Experience in hospice or a similar setting preferred
Responsibilities
- Assess/monitor physical, emotional, and psychological needs of patients
- Create hospice care plans that align with the patient's wishes and goals
- Direct nursing care: administering medications, treatments, and interventions
- Provide pain and symptom management
- Educate and support the patient’s family and caregivers
- Collaborate with interdisciplinary team
- Maintain accurate and timely documentation
- Participate in on-call rotation as required by local branch
