About The Position

Adoration Health is seeking a dedicated Hospice Registered Nurse Case Manager to provide patient-centered hospice care in Pittsburgh, Pennsylvania, with coverage in Highland Park and surrounding areas. The position is scheduled Monday through Friday, 8 a.m. to 5 p.m., and includes participation in an on-call rotation as required by the local branch. The RN provides care to patients and support to their families and caregivers, working independently while also collaborating with an interdisciplinary team.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least one year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Licensed driver with an insured automobile that is in good working order and meets state or organizational requirements.
• Current CPR certification in states where required.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create hospice care plans aligned with patients’ wishes and goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the on-call rotation as required by the local branch.
