Registered Nurse (RN), Case Manager Hospice
About The Position
Phoenix Hospice is hiring Registered Nurse Case Managers to provide compassionate nursing care to clients and families dealing with a terminal diagnosis. The role works with an interdisciplinary team to develop and implement patient- and family-centered plans of care, discuss goals of care, and guide healthcare decisions to support quality of life. The ideal candidate is energetic and motivated, with strong managerial, organizational, and interpersonal skills, and able to provide precise, effective guidance and support in demanding situations. Full-time and PRN availability is listed for Joplin, Missouri, and surrounding areas.
Requirements
- Missouri or Compact RN License.
- Valid driver’s license and auto liability insurance.
- Strong managerial, organizational, and interpersonal skills.
- High energy and motivation, with the ability to provide precise and effective guidance and support in demanding situations.
Responsibilities
- Educate patients and family members about the care process and plan, and guide them with healthcare decisions.
- Consult with the attending physician and Hospice Medical Director about changes in patients’ conditions that may require order or plan-of-care changes.
- Coordinate hospice care with team members and patients or family members to ensure needs are met.
- Conduct initial and ongoing comprehensive assessments.
- Perform evaluations for hospice care and informational visits.
