The Population Health Registered Nurse (RN) functions within the nursing scope of practice under the direction and supervision of the Provider (physician, PA or ARNP) and is responsible for assisting in providing care and implementing delegated activities of care. An understanding of contemporary nursing principles and skills must be demonstrated, unique to their clinical assignment. This role supports population health initiatives focusing on wellness, prevention, and chronic care management within the Family Medicine and Internal Medicine Departments. The RN collaborates, communicates, and coordinates with the Provider and other health care team members in planning and implementing patient care. They must understand and have knowledge of Medicare documentation, billing, and reporting requirements associated with Annual Wellness Visits (AWV), Chronic Care Management (CCM), and Transitional Care Management (TCM) services, including patient calls and calculation of billable time. The position involves performing face-to-face visits such as Medicare Annual Wellness Visits, conducting injection clinics (e.g., allergy injections, INR), performing tests as directed by a provider (e.g., EKG), and helping providers close patient care gaps (e.g., colonoscopies, mammography, A1c).
