Population Health Coordinator
About The Position
The Population Health Coordinator at Cherry County Hospital & Clinic in Valentine, Nebraska, supports patient health through preventive care, chronic disease management, and quality improvement initiatives. The role identifies and tracks patient care gaps, coordinates recommended services and follow-up appointments, monitors quality measures, supports value-based care and payer quality programs, and maintains patient registries and performance data. Requirements
- Understanding of quality measures, value-based care, HCCs, and payer programs.
- Strong analytical, communication, and organizational skills.
- Ability to manage multiple tasks while maintaining accuracy and confidentiality.
Nice To Haves
- Experience in healthcare quality, population health, care coordination, or clinic operations.
- Experience using electronic health records, Microsoft Office applications, reporting tools, and population health platforms.
Responsibilities
- Identify and track patient care gaps, including screenings, immunizations, and chronic disease follow-ups.
- Contact patients to schedule recommended services and follow-up appointments.
- Monitor quality measures and help improve patient outcomes.
- Support value-based care programs and payer quality initiatives.
- Maintain patient registries, review reports, and track performance data.
- Collaborate with providers, nursing staff, and leadership to improve workflows and quality of care.
- Educate patients about preventive services and available resources.
