About The Position

Cherry County Hospital & Clinic is hiring a full-time Population Health Coordinator in Valentine, Nebraska. The position supports improved patient health through preventive care, chronic disease management, and quality improvement initiatives. The coordinator identifies and tracks care gaps, contacts patients about recommended services, monitors quality measures and performance data, supports value-based care and payer initiatives, maintains patient registries, and works with clinical staff and leadership to improve care workflows. The role also includes educating patients about preventive services and available resources.

Requirements

• Understanding of quality measures, value-based care, HCCs, and payer programs.
• Strong analytical, communication, and organizational skills.
• Ability to manage multiple tasks while maintaining accuracy and confidentiality.

Nice To Haves

• Experience in healthcare quality, population health, care coordination, or clinic operations.
• Experience with electronic health records, Microsoft Office applications, reporting tools, and population health platforms.

Responsibilities

• Identify and track patient care gaps, including screenings, immunizations, and chronic disease follow-ups.
• Contact patients to schedule recommended services and follow-up appointments.
• Monitor quality measures and help improve patient outcomes.
• Support value-based care programs and payer quality initiatives.
• Maintain patient registries, review reports, and track performance data.
• Collaborate with providers, nursing staff, and leadership to improve workflows and quality of care.
• Educate patients about preventive services and available resources.
