About The Position

Capital Medical Group is a newly acquired physician practice group within Independence Physician Management (IPM), a subsidiary of UHS. We are seeking an experienced, strategic Human Resources leader to help shape the future of our organization and support the continued growth of a large, multi-specialty medical practice serving patients across the Washington, DC area and the DMV. The Nurse Navigator coordinates care to meet optimal outcomes for thoracic surgery patients across the care continuum. Within the Division of Thoracic Surgery, the Navigator coordinates referrals from providers and healthcare facilities, provides patient education, and serves as a resource for colleagues. Within the Division of Thoracic Surgery, the Lung Cancer Screening Nurse Navigator manages the daily operations of the GW low-dose screening CT program. Why Join Capital Medical Group? Requirements

  • Bachelor’s in nursing.
  • Current D.C. Licensure as a registered nurse or evidence of application for licensure in D.C.
  • A minimum of 2 years of experience as a registered nurse.

Nice To Haves

  • Experience in public health or health/patient education preferred.

Responsibilities

  • Manager for the GW Medicine Lung Cancer Screening Program
  • Keep up-to-date records/data for the program.
  • Perform all prescreening and screening for the LDCT program. Referrals are obtained from the 1-855-GWLUNGS hotline, via the EMR (Epic), and direct from providers.
  • Responsible for the shared decision making (SDM) counseling if not performed by the referring provider.
  • Offer and arrange smoking cessation counseling when appropriate.
  • Place LDCT scan orders and schedule the exam within our allotted slots.
  • Ensure results are sent to the referring provider and patient.
  • Review high-risk patients (LungRads 3 or 4) with Chief of Thoracic Surgery or designee.
  • Assist the Division staff with community outreach efforts as appropriate.
  • Assists the Clinical Nurse Navigator as needed.
  • Supports the vision, mission, and values of GW Medicine.
  • Provides and maintains a safe environment for caregivers, patients, and guests.

Benefits

  • Salary: $80,704 - $121,077
