About The Position
This PRN nursing position primarily serves the Pagosa Springs service area. CommonSpirit Health at Home uses AI-powered clinical documentation to reduce documentation time and support more direct patient care. The role emphasizes personalized, one-to-one care, meaningful patient relationships, and time to educate and support patients throughout their care. It also offers opportunities for professional development and potential growth into clinical leadership, operations, or education. The nurse provides high-quality, individualized care in patients’ homes; develops and adjusts care plans; coordinates with physicians, therapists, and support staff; and educates and supports patients and families. The role also includes overseeing a caseload and guiding LPNs, LVNs, and Home Health Aides, while using technology and best practices to support care delivery and accurate documentation.
Requirements
- Graduate of an accredited nursing program.
- Active RN license in the state served.
- At least 1 year of nursing experience.
- Exceptional critical-thinking skills and demonstrated clinical expertise.
- Strong case-management abilities and commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.

- Home Health experience is a significant plus.
Responsibilities
