About The Position

The RN Hospice coordinates and oversees all aspects of assigned patients’ hospice care. The role develops and implements individualized care plans in collaboration with the interdisciplinary team, conducts patient assessments, monitors symptoms, manages medications, and educates patients and families about conditions, needs, and care. The nurse communicates patient progress with clinical leadership, physicians, and the care team. This is a full-time, permanent-hire position offering full benefits and competitive compensation with a bonus structure.

Requirements

• Current Registered Nurse license.
• Current driver’s license.
• Current CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and valid automobile liability insurance.

Responsibilities

• Educate patients and families regarding condition, needs, and care.
