About The Position

Enhabit Home Health & Hospice is seeking an RN to provide hospice care in patients’ homes. The role coordinates care, continuously assesses the needs of patients and families, and implements the interdisciplinary plan of care in accordance with physician orders. Care must comply with applicable federal and state regulations. The position requires a current state nursing license, at least one year of experience as a licensed professional, basic technology skills, and the ability to meet driving and CPR requirements. Medicare home health or hospice experience is preferred.

Requirements

• Graduate of an approved school of clinical education.
• Licensed in the state where currently practicing.
• At least one year of experience as a licensed professional.
• Basic demonstrated technology skills, including operating a mobile device.
• Valid state driver's license.
• Maintain automobile liability insurance as required by law.
• Maintain dependable transportation in good working condition.
• Be able to safely drive an automobile in all types of weather conditions.
• Current CPR certification for the healthcare provider.

Nice To Haves

• Medicare home health or hospice experience is preferred.
• Experience as a licensed professional may be deferred with review and approval from regional and executive service line clinical leadership.

Responsibilities
