About The Position
CommonSpirit Health at Home is seeking a Hospice RN to provide personalized nursing care in patients’ homes, primarily in the Colorado Springs service area. The PRN position emphasizes meaningful patient relationships, dedicated time for care, and use of AI-powered clinical documentation tools. The nurse will assess patients, develop and adjust care plans, coordinate with a multidisciplinary team, educate and support patients and families, and guide LPNs, LVNs, and Home Health Aides. Requirements
- Graduate of an accredited nursing program.
- Active RN license in the state served.
- At least 1 year of nursing experience.
- Exceptional critical-thinking skills, clinical expertise, and case-management abilities.
- Commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.

Responsibilities
- Deliver high-quality, personalized nursing care in patients’ homes.
- Develop, implement, and adjust comprehensive care plans to promote patient independence and well-being.
- Collaborate with physicians, therapists, and support staff to coordinate holistic care.
- Educate and support patients and their families.
- Oversee the caseload and lead and mentor LPNs, LVNs, and Home Health Aides.
- Use clinical technology and best practices to optimize care delivery and maintain accurate documentation.
