RN Hospice
About The Position
As an RN Case Manager with Amedisys, part of the Optum family of businesses, you will coordinate compassionate, individualized hospice care. You will assess patients, work with the interdisciplinary care team, and develop care plans that address each patient’s needs. The position supports patients and families through hospice care in the home and emphasizes personalized care and trusted relationships. The role includes assigned on-call responsibilities. Requirements
- Current RN license specific to the state(s) assigned.
- Valid driver's license, reliable transportation, and liability insurance.
- Current CPR certification.
- At least 1 year of nursing experience.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records accurately and on time, in accordance with care center policies.
- Record changes and outcomes related to pain and symptom management as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Communicate regularly with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
