About The Position

CommonSpirit Health at Home is seeking a Hospice RN to deliver personalized nursing care in patients’ homes. The role emphasizes meaningful patient relationships, dedicated time for care, and use of AI-powered clinical documentation tools. Nurses develop and adjust individualized care plans, guide patients and families through care, collaborate with a multidisciplinary team, and oversee a caseload with support from LPNs, LVNs, and Home Health Aides. Requirements

• Graduate of an accredited nursing program.
• Active RN license in the state served.
• At least 1 year of nursing experience.
• Exceptional critical-thinking skills and proven clinical expertise.
• Strong case-management abilities and commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Nice To Haves

• Home Health experience is a significant plus.

Responsibilities

• Deliver high-quality, personalized nursing care in the patient’s home.
• Develop, implement, and adjust comprehensive individualized care plans that promote patient independence and well-being.
• Coordinate care with physicians, therapists, and support staff as part of a multidisciplinary team.
• Educate and support patients and their families, including providing emotional support.
• Oversee the caseload and lead and mentor LPNs, LVNs, and Home Health Aides.
• Use technology and best practices to optimize care delivery and maintain accurate documentation.
