About The Position

We are looking for a compassionate, talented, and experienced RN Hospice Nurse to join the organization’s Hospice team. The Hospice Nurse supports the personal, physical, emotional, and social needs of the patient-family unit. The role evaluates and responds to patient and caregiver needs through telephone contact, home visits, and/or facility visits, providing assessment, symptom management, treatment, teaching, and coordination of care. Full-time positions include a 7-on/7-off schedule, and Hospice Admissions openings are also available.

Requirements

• Active RN nursing license.
• Active CPR certification.
• At least 2 years of experience in hospice, palliative care, and/or training in end-of-life care.
• Knowledge of hospice care and services provided to patients and families/caregivers through an interdisciplinary group.
• Organizational skills.
• Effective verbal and written communication skills.

Responsibilities

• Evaluate and respond to patients’ and caregivers’ needs through telephone contact, home visits, and/or facility visits.
• Provide assessment, symptom management, and treatment.
• Teach patients and caregivers.
• Coordinate care.
• Support the personal, physical, emotional, and social needs of the patient-family unit.
