About The Position

As a Registered Nurse (RN), you will deliver compassionate, high-quality individualized care to hospice patients. You will perform assessments and skilled procedures, implement patient care plans, and collaborate with an interdisciplinary team to support patient well-being and outcomes. The role serves Shasta and Tehama Counties, with travel required to Tehama County. Call may be required.

Requirements

Associate degree in Nursing from an accredited school of nursing.
Current California RN license.
Verbal and written communication skills.
Computer skills.
Current California driver's license and automobile insurance.
Current BLS certification.
HPNA certification or hospice RN experience within the last two years. Training may be considered if no qualified candidates apply and operational conditions support training.

Nice To Haves

Bachelor of Science in Nursing degree from an accredited school of nursing.
Completion of ELNEC training.
Hospice and Palliative Care Nurse certification.

Responsibilities

Perform routine physical and psychosocial assessments to collect basic data; follow written plans of care, standards, practice guidelines, and pathways, notifying physicians as appropriate.
Formulate plans of care tailored to individual and patient-population needs.
Participate in planning routine transitional health care needs, including treatment and patient-placement options.
Adapt educational programs and information to individual patients and families, and integrate education into care delivery.
Promote efficient care delivery by identifying immediate priorities and offering solutions to problems.
Make independent clinical decisions based on education and experience.
Follow unit-specific safety regulations and guidelines.
Advocate for patient rights and identify potential conflicts; seek counsel to resolve conflicts.
Travel to Tehama County to serve patients in the assigned service area.
Respond to call when required.
