RN Hospice Nurse PRN
About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Per diem Hospice Registered Nurse provides compassionate, patient-centered care by assessing patient needs, coordinating individualized care plans, managing symptoms, educating families, and collaborating with the clinical team. The role includes on-call responsibilities and services to patients and families as assigned. The position offers development opportunities and performance-based recognition. The hourly pay range is $34.45 to $51.65, based on full-time employment, and pay is determined by factors including local labor markets, education, work experience, and certifications. Benefits and incentives are subject to eligibility requirements. Candidates must pass a drug test before beginning employment.
Requirements
- Current RN license specific to the state or states assigned for work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Nice To Haves
- At least 1 year of nursing experience.
Responsibilities
- Performs initial nursing assessments for new admissions.
- Completes, coordinates, and revises the total plan of care, maintaining continuity of patient care in collaboration with appropriate staff.
- Regularly communicates patient progress to the clinical manager, physician, and care team.
- Develops, prepares, and maintains accurate and timely individualized patient care progress records according to care center policies.
- Records pain and symptom management changes and outcomes as appropriate.
- Educates families about the patient’s condition, needs, and care.
- Makes referrals to other hospice care disciplines as needed, or documents the rationale for not doing so.
- Performs on-call responsibilities and provides on-call services to patients and families as assigned.
- Performs other duties as assigned.
