About The Position

Absolute Hospice, Inc., a family-owned agency, provides quality, compassionate care to patients in the comfort and privacy of their homes or places of residence. The Hospice RN establishes professional, caring relationships with patients and families, support staff, physicians, discharge planners, social workers, facilities, and other health care professionals. The role provides hands-on care, ongoing clinical management, and education regarding hospice care. The RN educates patients and caregivers about their conditions and the steps needed to ensure proper recovery.

Requirements

• Associate of Science degree in Nursing.
• Valid Ohio RN license.
• Minimum of 2 years of nursing experience.
• Valid driver’s license and one year of clean driving record.
• Excellent communication, writing, and charting skills.
• Excellent time management abilities.
• Independent problem-solving abilities and excellent clinical assessment skills.
• Computer literacy.
• Good physical shape.
• Ability to lift and transfer patients to and from wheelchairs, if necessary.
• Caring, patient, and flexible teaching approach.

Nice To Haves

• Hospice or home health experience is preferred.

Responsibilities

• Make initial nursing evaluation visits.
• Continually evaluate patients’ needs.
• Initiate, coordinate, and revise treatment plans.
• Render nursing care and perform treatments as ordered by the physician.
• Participate in the Quality Assurance/Quality Improvement process.
• Prepare accurate, orderly, and timely reports and documentation.
• Adhere to all practice standards as they apply to patient care.
• Educate patients and caregivers about their conditions and the steps needed to ensure proper recovery.
• Establish professional and caring relationships with patients, families, and health care professionals.
