RN Hospice Case Manager

About The Position
The RN Hospice Case Manager coordinates and oversees all aspects of assigned patients’ hospice care. The role develops and implements individualized plans of care with the interdisciplinary team, performs assessments, monitors symptoms, manages medications, and educates patients and families about patients’ conditions, needs, and care. The case manager communicates patient progress with clinical leadership, physicians, and the care team. This is a full-time, permanent-hire position with full benefits and a competitive compensation/bonus structure.
Requirements
- Current Registered Nurse license.
- Current driver’s license.
- Current CPR certification.
- Two years of Registered Nurse experience in a clinical care setting.
- One year of full-time hospice experience.
- Reliable transportation and valid automobile liability insurance.
Responsibilities

- Educate patients and families regarding condition, needs, and care.
