About The Position

Amedisys is hiring a full-time, day-shift RN Case Manager Hospice to provide hospice care to patients at home. The role includes assessing patients, collaborating with the care team on plans of care, maintaining patient records, supporting symptom management, and teaching patients, families, and caregivers. The position also includes assigned on-call responsibilities. The listed annual salary range is $88,000–$92,000; compensation may vary by geographic market, job-related knowledge, skills, and experience.

Requirements

• One year of nursing experience.
• Current RN license specific to the state or states where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain accurate, timely individualized patient care progress records according to care center policies.
• Record pain and symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Perform other duties as assigned.
