About The Position

As a Nurse Case Manager, you will guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, you will help ensure patients receive the appropriate level of care; provide education, coaching, and treatment-decision support; and collaborate with a Care Advocate and Social Worker. This role provides home health care and includes supervising LPNs and HHAs, communicating patient progress, and completing assigned on-call responsibilities. The position is full-time and focused on weekend work. Compensation includes per-visit-point pay for visits and hourly pay for certain non-visit activities. The stated annual total cash target pay assumes full-time employment at 40 hours per week and full productivity.

Requirements

Current RN license specific to the state(s) assigned for work.
At least 1 year of clinical experience as a Registered Nurse.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other disciplines based on identified patient needs, or document the rationale for not making referrals.
Promote patient health and independence through teaching and appropriate rehabilitative measures, including teaching self-care techniques.
Supervise LPNs and HHAs.
Complete documentation accurately and on time.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
