About The Position

As a Nurse Case Manager, you will guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, you will help ensure patients receive the appropriate level of care; provide education, coaching, and treatment-decision support; and collaborate with a Care Advocate and Social Worker. The role includes performing patient assessments, coordinating and implementing plans of care, teaching patients self-care techniques, supervising LPNs and HHAs, documenting care, communicating patient progress, providing staff education, and performing assigned on-call duties. You will also participate in clinical development and continuing education programs.

Requirements

Current RN license specific to the state(s) where assigned to work.
At least 1 year of clinical experience as a Registered Nurse (RN).
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other disciplines based on patients' identified needs, or document the rationale for not doing so.
Promote patient health and independence through teaching and appropriate rehabilitative measures, including self-care education.
Supervise LPNs and HHAs.
Complete documentation accurately and in a timely manner.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
