About The Position
CommonSpirit Health at Home seeks a Registered Nurse to provide personalized home health care in patients’ homes. The role emphasizes meaningful patient relationships, dedicated time for care, a 1:1 patient-clinician ratio, and guidance throughout the healing process. The nurse develops and adjusts individualized care plans, collaborates with a multidisciplinary team, educates and supports patients and families, and oversees and mentors LPNs, LVNs, and Home Health Aides. Requirements
- Graduate of an accredited nursing program.
- Active RN license in the state served.
- At least 1 year of nursing experience.
- Exceptional critical-thinking skills and proven clinical expertise.
- Strong case-management abilities and commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.

- Home Health experience is a significant plus.
Responsibilities
- Deliver high-quality, personalized nursing care in patients’ homes.
- Develop, implement, and adjust comprehensive individualized care plans to promote patient independence and well-being.
- Coordinate care with physicians, therapists, and support staff.
- Educate and support patients and their families.
- Oversee the caseload and guide LPNs, LVNs, and Home Health Aides.
- Use technology and best practices to optimize care delivery and maintain accurate documentation.
