About The Position

Requirements
- Graduate of an accredited nursing program.

- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.

Responsibilities
- Deliver high-quality, personalized nursing care in patients’ homes.
- Develop, implement, and adjust comprehensive care plans that promote patient independence and well-being.
- Coordinate care with physicians, therapists, and support staff.
- Educate and support patients and their families, including providing emotional support.
- Oversee the caseload and guide LPNs, LVNs, and Home Health Aides.
- Use technology and best practices to optimize care delivery and maintain accurate documentation.
