About The Position
CommonSpirit Health at Home seeks a Registered Nurse to provide personalized home health nursing care. The position emphasizes meaningful patient relationships, dedicated time for care, and a 1:1 patient-clinician ratio. The RN guides patients through the healing process, from initial visits to recovery, adapting care plans and supporting patient independence and well-being. Requirements
- Graduate of an accredited nursing program.
- Active RN license in the state served.
- At least 1 year of nursing experience.
- Exceptional critical-thinking skills and proven clinical expertise.
- Strong case-management abilities and commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.
Nice To Haves
- Home Health experience is a significant plus.
Responsibilities
- Deliver high-quality, personalized nursing care in patients’ homes.
- Develop, implement, and adjust comprehensive individualized care plans.
- Coordinate care with physicians, therapists, and support staff.
- Educate and support patients and their families.
- Oversee a caseload and guide LPNs, LVNs, and Home Health Aides.
- Use technology and best practices to optimize care delivery and maintain accurate documentation.
