About The Position
CommonSpirit Health at Home is seeking a Registered Nurse to provide personalized home health nursing care. The role emphasizes one-to-one patient care, meaningful time with patients, and guidance throughout the healing process. The nurse develops and adjusts care plans, supports patient independence, coordinates with a multidisciplinary team, and educates and emotionally supports patients and families. The RN also oversees a caseload and guides LPNs, LVNs, and Home Health Aides. The position uses technology and best practices to support care delivery and accurate documentation. This is a PRN position.
Requirements
- Graduate of an accredited nursing program.
- Active RN license in the state served.
- At least 1 year of nursing experience.
- Strong critical-thinking and clinical skills.
- Strong case-management abilities and commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.

Responsibilities
