About The Position
The Center for Disability Services is seeking a Registered Nurse Health Care Coordinator to join its Residential Division. The role is based at assigned residential programs, with a consistent caseload. Depending on the acuity and needs of the people supported, the RN may be assigned to one or two programs in the Capital Region area. The RN coordinates and oversees healthcare for the people supported, promoting health and individualized care planning. The role also includes providing training, education, and guidance to residential direct support professionals.
Requirements
- Hold a current New York State license as a Registered Nurse.
- Be able to lift a minimum of 50 pounds.
Responsibilities
- Assess healthcare needs and provide support and treatment to the people served.
- Follow the philosophy of the assigned Nursing Department and implement policies and procedures related to the delivery of nursing services.
- Collaborate regularly with team members, caregivers, and other professionals to ensure individuals' healthcare needs are met.
- Provide training, education, and guidance to residential direct support professionals.
