About The Position

Fairview is hiring a Care Coordinator Registered Nurse to work at its Clinics and Surgery Center in Minneapolis, primarily at the Maple Grove Specialty Clinic in Maple Grove, Minnesota. This is a full-time, salaried, 1.0 FTE position working Monday through Friday, with 8-hour shifts between 8:00 a.m. and 5:00 p.m. The position specializes in wound care in an ambulatory clinical setting. It provides expert clinical guidance, education, and support to patients, families, medical providers, and healthcare staff. The role promotes wound healing and prevention of complications through evidence-based assessment, treatment, and management. It coordinates patient-centered care across the continuum, and serves as a clinical resource and liaison to establish individualized treatment plans, support continuity of care, and achieve optimal wound-healing outcomes.

Requirements

Licensed Registered Nurse with a four-year baccalaureate degree.
Minnesota Registered Nurse (RN) license.
Basic Life Support certification from the American Heart Association or Red Cross. The posting states this requirement does not apply to department 8901 Ambulatory Care Coordination within 30 days.

Nice To Haves

Two years of specialty experience in care coordination, or five years as a licensed RN with five years of clinical experience and the ability to learn a new specialty area.
Specialized knowledge of the specialty and the conditions of the population served.

Responsibilities

Access, analyze, plan, and provide follow-up care.
Facilitate timely referrals and foster positive relationships with community providers.
Evaluate and document patient status, follow progress through phone calls and test results, and prepare reports.
Evaluate the quality of nursing care through patient rounds.
Identify problems specific to the clinical specialty.
Provide proactive disease management using evidence-based practices.
Act as a liaison among patients and families, medical staff, and others involved in treatment, promoting patient-centered care based on jointly determined goals and priorities.
Collaborate with appropriate staff to coordinate transitions of care across the healthcare continuum.
Provide direct patient care in the area of specialty.
Establish individual nursing care plans.
Provide education, counseling, medication management, and general health information to patients and their representatives.
Triage patient concerns and respond between visits under provider direction to reduce relapse or inpatient care.
Serve as a clinical-specialty resource to patient care units, outpatient clinics, and medical staff.
Identify the need for and assist with writing and implementing policies and procedures.
Provide education to patients, families, and hospital staff; develop content and resource materials and coordinate educational plans.
Serve on task forces and committees related to the clinical specialty.
Keep up to date on technological advances and new pharmaceutical products.
Consult with other areas regarding the specialty.
Assist with research projects, including data collection, analysis, and evaluation, and potentially identify research projects.
Manage databases to track patient outcomes and other specified categories as directed by a physician.
Promote the program and assist with marketing efforts.
Oversee or manage prior authorization and results verification processes to support a positive image.
Provide effective educational materials.
Serve as a community liaison.
Perform other duties, including performance goals developed by the manager and employee and reported in Fairview's performance management tools.
Develop and maintain the critical competencies listed in the job description and the employee's online performance management tool.
