HEALTH CARE COORDINATOR
About The Position
CTF Illinois is hiring a full-time Health Care Coordinator in the Mattoon/Charleston area. The position works directly with nursing staff to coordinate healthcare appointments for adults with developmental disabilities, accompany people served to appointments, and advocate for them. Work hours should coincide with Day Program operating hours, typically Monday through Thursday, 8 AM to 5 PM and Friday, 8 AM to 4 PM, but hours may change at a supervisor’s request. CTF Illinois provides community-based, person-centered programs and services for adults with developmental and intellectual disabilities, supporting individuals and their families with personalized planning and resources.
Requirements
- High school diploma or general education degree (GED).
- Basic user knowledge of Microsoft Office, including Excel, Word, and Outlook, and/or other PC applications.
Nice To Haves
- One year of work experience directly with individuals with developmental disabilities is preferred.
- Previous DSP certification is preferred.
Responsibilities
- Check monthly for residents’ appointments that need to be scheduled, including medical, vision, dental, and neurological appointments.
- Schedule and confirm appointments, coordinate transportation, and accompany individuals to all appointments.
- Follow up as necessary and appropriate after appointments.
- Assist nursing staff with checking in monthly medications when requested or needed.
- Ensure that a medical synopsis form completed by a nurse is taken to appointments when appropriate.
- Return appropriate documentation to nursing staff for follow-up.
- Ensure physician orders are signed by a physician monthly.
- Complete the bi-annual medication review form and obtain the physician’s signature.
- Adhere to the terms and conditions of employment outlined in the Employee Handbook and job description.
