RN Field Case Manager in Biloxi, MS
About The Position
Optum Home & Community Care, part of the Optum family of businesses, is building an integrated care model that addresses patients’ physical, mental, and social needs. As part of the Optum Care at Home team, the RN Field Case Manager works in an interdisciplinary care environment to help patients navigate the health care system and connect with support services. The program coordinates longitudinal clinical care for members in their residences. The DSNP program provides intensive, individualized interventions in collaboration with the Interdisciplinary Care Team, including clinicians, members’ Primary Care Providers, other providers, and professionals. This field-based position serves members throughout Biloxi, Mississippi. The schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m. EST, though hours may vary based on business needs. The role involves visiting members at home or in local long-term care facilities.
Requirements
- Current, unrestricted RN license in the state of Mississippi.
- Certification in Basic Life Support.
- At least 1 year of experience working in community or public health nursing.
- Reside in or near Biloxi, Mississippi.
- Valid driver’s license and access to reliable transportation.
Nice To Haves
- Bachelor of Science in Nursing (BSN).
- Certified Case Manager (CCM).
- Case management experience.
- Home health experience.
- Background managing populations with complex medical or behavioral needs.
- Bilingual in English and Spanish.
Responsibilities
- Visit members in their homes to assess their current health status.
- Identify gaps or barriers in treatment plans.
- Provide patient education to assist with self-management.
- Make referrals to outside sources.
- Provide a continuum of quality care through close communication with members in person, at home, or by phone.
- Support members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels.
